What is on the form
- Owner and purpose (insurance, sale, estate).
- Item: type, maker, metal, karat, gross weight in grams, size, hallmarks and a free-text description.
- Stones: up to four stones with shape, carat, color, clarity and origin or treatment, plus lab and certificate number.
- Condition and notes on wear, repairs, loose or missing stones.
- Value: the basis (retail replacement, fair market or liquidation), the amount, the date and the gold price used, plus which photos were taken.
- Signatures of the appraiser and the owner.
Three habits that make an appraisal hold up
- State the basis. A retail replacement value for insurance is much higher than a liquidation value. Always tick which one you mean.
- Photograph before you write. Front, back, hallmark and a stone close-up. Keep the photos with the form number.
- Date everything and note the gold price used. The same ring is worth different amounts in different months.
When paper stops scaling
If you issue many of these, retyping the same item data on every form wastes time. Keeping the piece in your inventory with its karat, weight, stones and certificate number lets you print the details instead of rewriting them. See the free inventory spreadsheet, or the gold price calculator for the metal part of a value.
Want this done for you?

Frequently asked questions
What should a jewelry appraisal include?
Owner details, a full item description (type, metal, karat, weight, measurements, hallmarks), every stone with carat, color, clarity and treatment, lab certificate numbers, condition, photos, the basis and amount of value, the date, and the appraiser's signature.
Is this form a certified appraisal?
No. It is a template for recording a description and an opinion of value. Rules and qualifications for appraisals used for insurance, estates or court differ by country and state. Check yours before issuing one.
How often should insured jewelry be re-appraised?
A common rule is every two to three years, sooner if gold or diamond prices move sharply.
